Practitioner Registration Form
Please read ALL of the following CAREFULLY:
- Holisto is an Irish Database, we cannot validate submissions from outside
of Ireland and Northern Ireland.
- An email address is required for registration. You can sign up for a
free email address at www.gmail.com among
other places.
- If you are already registered, you can currently only update your entry
by email. See www.holisto.com/contact.php
- Free Trial and charges:
- There is no obligation to continue after the free trial.
You cannot have a second free trial after the first. Basic subscription is normally 50 euro per annum.
- Security Key: Due to abuse of our registration
form the entry of a security key is now required. For the security key please
send an email to registration@holisto.com and it will be sent to you automatically
by a machine. Your email address will not be used for any other purpose,
or given to a third party. To get the security key you need to email that
address: registration@holisto.com.
- If you do not received automated reply from that address, please check that
it has not been deleted as spam.
- By submitting an application to be listed you agree to the following terms
and conditions: *You agree to receive email communications from holisto.com,
for the duration of your listing. (these are currently about once a year,
but could become more frequent) *You agree to receive contacts through our
contact forms. These forms do not display your email address.
*Abuse of the contact forms for commercial purposes will result in your removal from the site.
* Listing of your website, if applicable, is on a reciprocal basis. We reserve
the right to discontinue your listing.
* Updates are currently by email. Contact
us with your required changes, reminding us of your full name, location
and therapies.
- In
normal circumstances we require that either a landline phone number, or a
full postal address be supplied for our own records. This ensures traceability,
and is a protective measure for our users.
- We reserve
the right to refuse publication solely at our own discretion, subject to legal
limits.
